Frank Farnsworth

BRANDON — Frank A. Farnsworth, 90, died Saturday, Jan. 30, at the home of his son in Brandon. He was born June 4, 1919, in Manchester, N.H., the son of Frank A. and Claudine (Miller) Farnsworth. 

At his request, there will be no formal services. He donated his body to the Dartmouth Medical School and, when his final teaching assignment is completed, family and friends will take him to join both Ruth and Libby, who await him on Mount Chocorua in his beloved White Mountains of New Hampshire.

David Douglas

MORIAH, N.Y. — David “Mike” Douglas, 72, of Moriah died Sunday, Feb. 7, 2010, at Elizabethtown Community Hospital following a 3-year battle with pancreatic cancer.

A funeral service was held Tuesday, Feb. 9, 2010, at the Harland Funeral Home in Port Henry. Burial will be at a later date in Prospect Cemetery in East Middlebury, Vt.

Memorial donations may be made to the Shriners Children Hospital.

Richard Brock


EAST MIDDLEBURY — Richard W. Brock, 86, died early Sunday morning, Feb. 7, 2010, at his residence in East Middlebury.

A funeral service was held Wednesday, Feb. 10, 2010, at Sanderson-Ducharme Funeral Home in Middlebury.

Burial will be at a later date.

He was born Feb. 21, 1923, in Auburn, N.Y., the son of the late Walter B. and Mabel (Chapin) Brock.

He attended local schools in the Mountain Lake Park, Md., area and was a graduate of Middlebury College.

BRANDON — Robert John Atwood, 81, died Thursday, Feb. 4, 2010, at Rutland Regional Medical Center.

Ina Jewell

MONKTON — Ina Jewell, 95, died Wednesday, Feb. 3, 2010, at Porter Medical Center in Middlebury.

Funeral services were held Saturday, Feb. 6, at Brown-McClay Funeral Home in Bristol. A reception followed at Monkton Central School. Interment will be at a later date.

Memorial contributions may be made to Bristol Rescue Squad, PO Box 227, Bristol, VT 05443.

She was born July 13, 1914, in Jericho, the daughter of Quincy and Harriett (Burritt) Thurston.

John Quesnel

BAREFOOT BAY, Fla. — John Louis Quesnel, 92, of Barefoot Bay, Fla., died Wednesday, Feb. 3, 2010, at Sebastian River Medical Center in Sebastian, Fla., after a courageous battle with emphysema.

There will be no calling hours and the service will be private.

He was born on Feb. 14, 1917, in Moose Creek, Ontario, Canada, to Blanche and Eli Quesnel. Shortly after his birth, the Quesnel family moved to Leicester, Vt.

Samuel Paretti

MIDDLEBURY — Samuel Paretti, 99, died Sunday, Jan. 24, 2010, at The Lodge in Middlebury. At his side were his daughter, Lynne Smaldone, his son, Michael Paretti, and his loving wife of 76 years, Francette.

Helene Kolbus

WATERMILL, N.Y. — Helene V. Kolbus of Watermill, N.Y., sister of Julie S. Keeler of Middlebury, died Saturday, Jan. 23, 2010, after a brief illness.

Services were held Jan. 29 at Our Lady of Poland Church in South Hampton, N.Y.

Interment will be at St. Mary’s Cemetery in Jewett City, Conn., at a later date.

Sonny Cyr

MIDDLEBURY — Sonny Cyr of Middlebury died peacefully at Birchwood Terrace Jan. 27, 2010.

Services will be held Saturday Feb. 6, at 10 a.m. at St. Mary’s Church in Middlebury. Interment will follow at St. Peter’s Cemetery in Vergennes.

Donations in lieu of flowers can be sent to the Middlebury Fire Department, 5 Seymour St., Middlebury, VT 05753.

Sonny was born in Weybridge on March 25, 1942, son of Leonard P. and Doris L. Cyr.

He enjoyed being in the woods, hunting and fishing.

Mary Butterfield

MIDDLEBURY — Mary C. Butterfield, 84, of Middlebury died Thursday morning, Jan. 28, 2010, at Helen Porter Healthcare and Rehabilitation Center.

A memorial service will be held in the spring at St. Mary’s Catholic Church.

In lieu of flowers memorial contributions may be made to St. Mary’s Catholic Church, 326 College St., Middlebury, VT 05753.

She was born Nov. 30, 1925, in Newark, N.J., the daughter of the late George and Emma (Perkins) Coursey.

Submitting an obituary

The Addison Independent publishes paid and free obituaries. Paid obituaries run just as submitted with a charge of $9.00 per column inch, and the editor shapes free obituaries to conform to our editorial style. We run photos of the person if they are submitted in a relatively high-quality format.

Generally, obituaries follow this form:
TOWN - Name, age, of town, died on date, of cause of death. He/She was born on date in town, the son/daughter of father's name and mother's name, including maiden name in parentheses.

Then include a paragraph or two about where the person went to school and what they did for work; where they lived, who they married and when.

Add a sentence or two about what made the person special to family and friends, including hobbies, avocations or other telling details about their life.

He/She is survived by spouse's name; children named individually (and their spouses and towns of residence, if appropriate); siblings named individually; number of grandchildren, parents or other immediate family members.

He/She was predeceased by names of immediate family members.

A memorial service will be held (fill in details of date, time, and place, plus officiate if applicable) with interment at (place). Memorial contributions may be made to (name and complete address, if applicable).
