RAYMOND OUELLETTE

SHOREHAM — Raymond Joseph Ouellette, age 64, officially known as “Rayzin” by his friends, died Friday, Dec. 6, 2013, at Helen Porter Healthcare & Rehabilitation Center in Middlebury.

GIACOMINA ROY

RUTLAND — Giacomina Marisa Roy, 85, of Rutland died Monday, Dec. 9, 2013, at her home in Rutland.

She was born in Rome, Italy, on May 12, 1928, the daughter of Mr. and Mrs. Fuseti. She moved from Italy to Rutland in 1958.

She was married to Charles Blake Roy Sr., who died years ago.

She was a seamstress, employed at Dick’s Dress Shop and Linda Lee Fashions. She was a maker and collector of dolls and a member of the Foster Grandparent Program.

PAMELA SHELDRICK

WHITING — Pamela M. Sheldrick, 55, died Saturday Dec. 7, 2013, at Rutland Regional Medical Center. 

Born in Rutland Aug. 29, 1958, she was the daughter of William E. and Mary S. (Archambeault) Sheldrick Sr.

She was a homemaker and had previously been employed by the Shoreham Co-op for over 20 years, the Whiting Country Store, and Vermont Tubbs of Brandon. She had lived in Whiting since 1994.

SHIRLEY WILLIAMS

RIPTON — Shirley E. Williams, 76,  of Ripton died at Fletcher Allen Health Care on Dec. 10, 2013, after a long illness.

She was born March 23, 1937, the daughter of L.D. and Florence Williams.

She worked as a logger and farmer with her father. Later she worked at Middlebury College on the Bread Loaf campus as head maid.

She is survived by her brother, Donald “Pete” Williams and his wife Carol; a sister-in-law, Betsy; and many cousins, nephews and nieces.

JAMES C. BORDEN

MIDDLEBURY — James “Jim” C. Borden, 85, of Middlebury, and formerly of South Bend, Ind., died on Dec. 1, 2013.

He was born on Oct. 14, 1928, in Indianapolis, to James G. and Anna (Hasewinkle) Borden. He graduated from Adams High School in 1947 in South Bend, and served in the U.S. Army from 1951-1953. He also attended the School of the American Academy of Art in Chicago.

HELEN KINGMAN

ST. ALBANS/MIDDLEBURY — Helen Naylor Kingman, 96, died Thursday morning, Dec. 5, 2013, at St. Albans Healthcare and Rehabilitation Center.

She was born Aug. 2, 1917, in Middlebury, the daughter of the late William C. and Faith (McCray) Naylor. She attended local schools in Middlebury and married her husband, Raymond Kingman, Dec. 31, 1936.

She was of Methodist faith. Her family says she was an avid bird watcher and also liked to watch squirrels.

She and her husband owned and operated Kingmans Market in Middlebury for 17 years.

LAURA PONDER

MIDDLEBURY — Laura Grace Ponder, 38, of Middlebury died Nov. 8, 2013, from complications resulting from prior cancer treatment. She was being visited and comforted by her family at the time of her death. She was born March 2, 1975, in Poughkeepsie, N.Y., to Jim and Cynthia Ponder.

She was raised in New York and North Carolina and graduated from Watts School of Nursing in Durham, N.C. She made Vermont her home appreciating the kindhearted and progressive community that welcomed her here.

KATHLEEN PROVONCHA

SARATOGA SPRINGS, N.Y. — Kathleen A. Provoncha, 65, died on Sunday, Dec. 1, 2013, after a long struggle with cancer.

She was born in Mineville, N.Y., on May 6, 1948, to the late Patrick J. and Marie Little Coyle. She graduated from Vergennes Union High School in Vermont and retired from NYS Civil Service in Albany, N.Y. She was a parishioner of St. Clement’s Church in Saratoga Springs and served on the board of the Christian Women’s Club. She was active in various women’s retreats and Bible studies. She also was a Weight Watchers lecturer.

BARBARA SKOGSTROM

MIDDLEBURY — Barbara C. Skogstrom, 84, died Wednesday, Dec. 4, 2013, at Helen Porter Healthcare and Rehabilitation. Born in Worcester, MA, June 14, 1929, she was the daughter of the late John A. and Anna A. (Juthstrom) Carlson.

Barbara graduated as a registered nurse from Worcester City Hospital in 1950, and worked in many different healthcare settings. In addition she enjoyed sewing, knitting, needlepoint and floral arranging. She was a member of the Crown Point Methodist Church.

JUDITH M. (COYLE) HART

MONKTON — On Dec. 1, 2013, Judith M. (Coyle) Hart was unexpectedly taken to live eternally with the Lord. She was born Oct. 28, 1958, in Ferrisburgh. She enjoyed her large family, helping others, gardening and the outdoors. Judith worked many years at Digital Corp. and then Autumn Harp and Country Home Products.

Submitting an obituary

The Addison Independent publishes paid obituaries and free notices of passing. Paid obituaries cost 25-cents per word, and $10 per photo. The obituary will publish as it is submitted on the date of the family’s choosing.

For those who do not wish to purchase an obituary, the Addison Independent offers anyone with a local connection the opportunity to publish a notice of passing. These notices may be up to 100 words and are subject to editing by our news department.

Submissions must be received by 11 a.m. on Wednesday for publication in the Thursday paper and 11 a.m. on Friday for Monday’s paper.

Generally, obituaries follow this form:
TOWN — Name, age, of town, died on date, of cause of death. He/She was born on date in town, the son/daughter of father's name and mother's name, including maiden name in parentheses.

Then include a paragraph or two about where the person went to school and what they did for work; where they lived, who they married and when.

Add a sentence or two about what made the person special to family and friends, including hobbies, avocations or other telling details about their life.

He/She is survived by spouse's name; children named individually (and their spouses and towns of residence, if appropriate); siblings named individually; number of grandchildren, parents or other immediate family members.

He/She was predeceased by names of immediate family members.

A memorial service will be held (fill in details of date, time, and place, plus officiate if applicable) with interment at (place). Memorial contributions may be made to (name and complete address, if applicable).
