PAGE UFFORD JR.

WEST BRANDYWINE TOWNSHIP, Pa. — Page S. Ufford Jr., 92, of West Brandywine Twp., formerly of Newark, Del., and Middlebury, Vt., passed away on Sept. 22, 2013.

Born in Middlebury, Vt., in 1921, Page was the son of Ildah (Gale) and Page S. Ufford, Sr., both deceased.

PATRICIA CHAMBERLAIN

STARKSBORO — Patricia (Grant) Chamberlain, 68, of Starksboro, died on Sept. 19, 2013, in Burlington, after a long and courageous battle with heart and vascular disease. She was born on April 23, 1945, in Hancock, the beloved daughter of Edward and Helen (Faivre) Grant. She was married on May 19, 1962, in Bristol to Thomas Chamberlain.

DONALD FORTUNE

RUTLAND — Donald Adam Fortune, 51, died Wednesday, Sept. 18, 2013, at his home in Rutland.

He was born March 25, 1962, in Avon, Conn., to Gerard and Virginia (Stupcenski) Fortune. He received his education in Leicester schools and at Otter Valley Union High School in Brandon. He married Dawn Crickmore on Oct 27, 1987, and lived in the Rutland area for many years.

He was employed by Terry Hill Trucking in Milton. His family says his interests included outdoor cooking on the grill, bargain shopping and fishing.

BETTY GODFREY

BRANDON — Betty Godfrey, 75, of Brandon died Sept. 11, 2013, after a short battle with cancer

She was born Bessie Louise Godfrey on Sept. 17, 1937, to Arthur and Florence (Webster) on the family farm in West Fairlee where she grew up with three brothers and two sisters and a large extended family of aunts, uncles and cousins.

LILIAN AYE

NORTH FERRISBURGH — Lilian Aye, 85, of North Ferrisburgh, died on Aug. 24 at the Vermont Respite House in Williston.

She was born in Germany where she was a writer and journalist. She came to the United States and worked in the hotel and restaurant business in Stowe and later in Middlebury where she managed Rehearsals Café at Middlebury College.

JOSEPHINE COLE

CHIMNEY POINT, Vt. — Josephine Rippey Cole, 77, died of cancer at her home at Chimney Point in West Addison on Sept. 13. She was born on Jan. 10, 1936, in Syracuse, N.Y., the daughter of Harold Rippey and Mildred (Post) Rippey.

Jo was a graduate of Nyack (N.Y.) High School and Heidelberg College in Tiffin, Ohio. She earned a master’s degree in special education from Lehman College in New York and for over two decades was an elementary and middle school teacher in the South Orangetown Central School District, beloved by students and colleagues alike.

RAY W. "DOC" COLLINS JR., MD

MIDDLEBURY — Middlebury lost one of its best last Saturday, Sept. 14, 2013. Ray was born in Brookline, Mass., on June 20, 1914, to Ray and Lillian Collins while Ray Sr. was still pitching for the Boston Red Sox. They moved to Colchester, Vt., in 1915 to operate the family farm. While growing up, he helped his father on the farm and raised chickens. Ray attended elementary school in Colchester and graduated from Burlington High School in 1931.

BRANDON —Tina Marie Forrest, 57, died Monday, Sept. 16, 2013, at her home in Brandon.

She was born in Rutland on March 4, 1956. She was the daughter of Raymond and Elizabeth (Ryder) Forrest. She received her early education in local Forest Dale and Brandon schools and attended Otter Valley Union High School.

MIDDLEBURY — A celebration of life service for Ruth E. Lennox, 94, who died July 29, 2013, will be held at St. Stephen’s Episcopal Church in Middlebury on Saturday, Sept. 21, at 11 a.m.

EDWARD F. “DICK” MENARD

LINCOLN — Edward F. “Dick” Menard, 78, one of the most unique and kind men ever known, was lifted away quietly on Sept. 13, 2013, while he slept surrounded by his family at his home in Lincoln, Vt. He was loved by many, his family will miss him, and may God greet him with open arms.

Submitting an obituary

The Addison Independent publishes paid obituaries and free notices of passing. Paid obituaries cost 25-cents per word, and $10 per photo. The obituary will publish as it is submitted on the date of the family’s choosing.

For those who do not wish to purchase an obituary, the Addison Independent offers anyone with a local connection the opportunity to publish a notice of passing. These notices may be up to 100 words and are subject to editing by our news department.

Submissions must be received by 11 a.m. on Wednesday for publication in the Thursday paper and 11 a.m. on Friday for Monday’s paper.

Generally, obituaries follow this form:
TOWN — Name, age, of town, died on date, of cause of death. He/She was born on date in town, the son/daughter of father's name and mother's name, including maiden name in parentheses.

Then include a paragraph or two about where the person went to school and what they did for work; where they lived, who they married and when.

Add a sentence or two about what made the person special to family and friends, including hobbies, avocations or other telling details about their life.

He/She is survived by spouse's name; children named individually (and their spouses and towns of residence, if appropriate); siblings named individually; number of grandchildren, parents or other immediate family members.

He/She was predeceased by names of immediate family members.

A memorial service will be held (fill in details of date, time, and place, plus officiate if applicable) with interment at (place). Memorial contributions may be made to (name and complete address, if applicable).
